Drop Down Optin Box Creation

Hi,

I use aweber to collect email adress of my visitors and i
need a new drop down box. Actually the box is cheap with no
image of my product. I want something better and more flashy.

Here is what i want:
- Opt in box design with the text i provide
- Use element of my website (ebook cover and logo)
- .jpg + .psd of the box send to me for future mods
- Aweber html form ready to insert on the box
