Capture Flyout Menu Code/css

Hi,

We need a JS and CSS guru to capture the JS and CSS from here http://tinyurl.com/25dh7u to recreate this menu (see top flyout menu)and provide this to us for our use on our website

We need:
1. One JS file with required Flyout JS code (other libraries as needed also ie jquery)
2. One CSS file with required styles
3. One HTML file with working Flyout Menu
3. All menu graphics – backgrounds/gradients etc
4. Instructions on implementing this within a web page (onload, class names etc)
