Form should also use AJAX.

Form should contain the following fields.

Last Name:
Email:
Phone:
Subject:
Message:
Spam Protection:
* All fields should be required.
Form should be sent to our email address.
Spam Protection should use CAPTCHA
Form should use our CSS file.
