I have a subscription form for my newsletter. It saves names & email address in to database, based on category. There are 4 categories with various subcategories in each. HERE IS PROBLEM: The form only saves email address and not name… (Budget: $30-$250 USD, Jobs: HTML, MySQL, Smarty PHP)
