WordPress Form with Paypal Plugin by thefunstarts

I run a website and need a wordpress plugin which will allow users to write in information. Heres how I want it to work: I want to be able to define the fields. Ability to define input boxes i.e Drop down menu, Multiple Line input box, paragraph box, Radio buttons, Checkboxes… (Budget: $30-$250 USD, Jobs: HTML, PHP, Software Architecture, WordPress)
