Adding Point, lines and polygons to exsiting Map by jjconcepts

I need a single ArcGIS Javascript API page written that will use an existing page/menu webpage but to incorporate the ability to add/edit a point, line and/or polygon save into two field parameters (MCDNUM and Name), both taken from the URL string… (Budget: $30-$250 USD, Jobs: Cartography & Maps, Geolocation, Geospatial, Javascript)
