I need help to create a forms for our WordPress website please see the files attached. I don’t seem to find a correct plugin for it. when a user login they can fill this form send it to an email as excel spreadsheet… (Budget: $30-$35 USD, Jobs: CSS, HTML, PHP, WordPress)
